Street Lane Practice approach

As a GP Practice we are now undertaking a range of work to support the government response to the coronavirus outbreak. Please click here to view more information on how we will process your personal data in the course of this work. You can also click here to view our full COVID-19 privacy notice.

At this time please do not attend the practice, unless you are advised to do so by a member of the Practice team. If you are asked to attend the practice, you will be kept separate from other patients and the Dr will be wearing full PPE. Please note all patients who are required to attend the practice will be asked to wear a face covering. 

Based on Government guidance, our approach at Street Lane Practice approach is: 

 

In general we will try to switch all possible appointments to telephone triage or video consultation.

 

If a patient needs to attend the surgery for face to face review he/she will be screened for flu-like symptoms. If these symptoms are present you will be seen by a GP in protective gear in a designated consultation room. If these symptoms are not present you will be seen at the surgery in the usual way.

Call Recording 

Dear Patients,

 

We would like to inform you of an update to our Practice.

 

All inbound telephone calls will be recorded to:

 

  •        Protect the interests of both parties

  •        Help improve practice performance and service delivery in the interests of providing best care

  •        Protect Practice team from nuisance or abusive calls 

  •        Establish facts relating to incoming/outgoing calls made (e.g. concerns, complaints and medico-legal claims)

Contract compliance as part of Contemporaneous Record Keeping part of Records Management Policy and Access to Health Records

 

Outbound calls WILL NOT routinely be recorded; though if a staff member feels that this would be appropriate the patient will be asked for permission prior to recording.

Travel Vaccinations

If you are travelling abroad, please use the 'Fit for Travel' website Home - Fit for Travel for travel advice and the recommended vaccinations for your chosen destination. Street Lane Practice can only administer the following vaccines for travel under the NHS.

  • Hepatitis A

  • Typhoid 

  • Diphtheria / Tetanus / Polio

For the following private vaccinations

  • Yellow fever

  • Japanese Encephalitis

  • Tick-Borne encephalitis

  • Varicella ( Chickenpox)

  • Rabies

  • Hepatitis B.

Patient Forum

At Street Lane Practice we have established a Patient Representative Group, which enables our patients to become more involved with the service we provide. Meetings are held at intervals throughout the year to gauge feedback and opinions on various aspects of our service. If you would like to be included in our Patient Representative Group please contact the practice manager- Helen Dickinson.

Register as a patient: the catchment area

The practice is happy to register new patients living within our catchment area (see below). If you are already a patient and are moving locally please contact reception for advice. You may still be eligible to stay with the practice.

 

​Registration usually takes up to 48 hours to be put on the system. After this you will be able to book appointments. Each family member will need to register individually. If you need any assistance please ring reception.

GDPR

The current NHS Digital (NHSD) extract of GP data for Research purposes (known as the GPDPR) has been delayed due to NHS Digital wishing to review the way in which this data will be collected, to conduct more public involvement and information about the plans and change the way in which patients can opt out of the extract of their GP data.
