Keep your information with us up-to-date
Increasingly we will be using mobile phones and email addresses to contact our patients to arrange chronic disease review and to communicate test results. We encourage patients to ensure that their contact details registered with us are up to date (update contact details).
Additionally we encourage patients to communicate clinical information about themselves annually such as height, weight, blood pressure and smoking status so that we can monitor physical health (update clinical details).
Our premises offer easy access with all patient facilities on the ground floor. There is a ramp and disabled parking spaces at the front entrance. We have disabled toilets with baby changing facilities and a hearing loop that can be used in all the clinical rooms.
We have a branch of Lloyds Chemist within our premises.
The practice closes for essential staff training on Thursday afternoon usually at 6 weekly intervals.
Forthcoming training dates are:
14/1/16, 11/2/16, 10/3/16, 21/4/16, 19/5/16, 23/6/2016, 14/7/16, 22/9/16, 13/10/16, 10/11/16
Information will be provided on our front doors and the telephone answering machine as to how to access urgent medical care when we are closed.
Sorry for any inconvenience.
Our specialist clinics provide consultant-led, hospital-quality treatment at your local GP practice. For patients this can be quicker and more convenient than attending a hospital.
In the interest of full disclosure it should be noted that Street Lane Services is part of the Street Lane Practice. Therefore the partners at Street Lane Practice make a small profit from the clinics. Patients registered at Street Lane Practice can be referred to our internal clinics or they can choose to be referred to alternative clinics in hospital for instance.
Out of hours emergencies
Patients with urgent medical problems which occur when the surgery is closed either from 6:30pm or at the weekends and Bank Holidays should telephone 111. They will be directed to the most appropriate service.
Nearest Walk-in Centre Burmantofts Medical Centre, Cromwell Mount, Leeds, LS9 7TA 0113 295 1132.
From mid November 2014 we will be opening Street Lane Practice on 2 evenings every week to improve access to our clinicians. This is in response to the extremely positive feedback we've received about the existing weekly evening surgery.
